MedBen Platforms Privacy Policy
Overview:
The MedBen Access site (including the MedBen Access website and the MedBen Access mobile application – collectively the "MedBen Platforms") is designed to provide access to tools and information to help you manage your health plan benefits. MedBen understands that you expect us to protect both your medical (protected health) information and your personal (individually identifiable) information. We respect your right to privacy and your expectation that all information about you will be kept in a secure manner and are committed to maintaining the confidentiality, security and integrity of your information. This Privacy Policy explains MedBen’s policies and procedures for handling your information.
User Authorization:
This Privacy Policy applies only to information contained and collected through the MedBen Platforms. This includes the registration process which is also used to create your electronic signature, verification, acceptance and approval. You must complete the registration process and accept MedBen terms (Terms of Acceptance) for using either of the MedBen Platforms prior to obtaining access to them. To complete the registration process, MedBen Access users are required to provide answers to user validation questions. This information is used to authenticate your identity.
Changes to our Privacy Policy and Terms of Acceptance:
MedBen reserves the right to modify or remove parts of this Privacy Policy or its Terms of Acceptance at any time. It is your responsibility to check for, review and understand any updates or revisions to the privacy policy or terms of acceptance.
Sharing Information:
MedBen will not disclose any of your information except:
1. as described in the Terms of Acceptance;
2. as described in your health plan’s HIPAA privacy policy, your health plan document and your health plan’s Notice of Privacy Practices;
3. as described in any authorization signed by you;
4. with your permission
5. as required and/or allowed by a Business Associate Agreement into which MedBen has entered; and
6. as required by law...
The conditions under which this information is released is described in the Terms of Acceptance. In some cases, de-identified information may be shared as aggregated data with those assisting in the administration of your health plan or for other lawful purposes.
Security of Your Information:
MedBen is committed to protecting the privacy and security of the MedBen Platforms during the transaction process. Our Internet infrastructure is protected using strong commercial security products and is monitored. MedBen maintains physical, electronic and procedural safeguards to protect your information.

The MedBen Platforms contains links to other websites. MedBen is not responsible for the privacy practices or the content of such websites.
Cookies:
In order to provide you with tailored and personalized services, MedBen may use "cookies" to store and track your information. A "cookie" is a small amount of data that is sent to your browser from a web server and stored on your computer’s hard drive. Some pages of the MedBen Platforms use cookies so that MedBen can better serve you when you return to this site at a later date. You can set your browser to notify you when you receive a cookie, giving you the chance to decide whether to accept it. You should also know that if you change the settings on your computer or your mobile phone you may limit or interrupt your access to, or ability to obtain data from, the MedBen Platforms.

Terms of Acceptance / Electronic Signature:
The IDs and passwords used to access the MedBen Platforms are created by completing the registration process that occurs prior to accessing either the website or the mobile application. As part of the registration process, you are required to provide certain personally identifiable information, including, but not limited to your personal identification number, name, birth date and zip code. This information is collected in order to verify and identify you later. By completing the registration process, you agree to the following, without exception:
I hereby request coverage and authorize that any requested contribution for the health plan coverage or insurance to which I may be entitled be deducted from my earnings. I am employed by the employer shown and am working at least the number of hours per week required by my Employer and/or shown on the Employer Application. I authorize (1) any physician, hospital, or other health practitioner or facility, (2) any insurance company or health care plan, (3) any state or federal agency providing health care benefits, and (4) any employer to provide Medical Benefits Mutual Life Insurance Co. or Medical Benefits Administrators, Inc. ("MedBen") or its legal representative any information in their possession which is relevant to any application for insurance or health plan coverage regarding myself or my applicable Dependent(s). This information will be used to determine the eligibility for coverage and/or benefits for myself and my applicable Dependent(s) and will be utilized by employees, agents and business associates of MedBen with responsibility for (1) reviewing applications and determining eligibility for coverage, (2) payment of claims, and (3) any other health care operations. I hereby authorize and release any provider of health care services, claim administrators, insurers, reinsurers, pharmacy benefit managers, and other business associates who have a legitimate need for such information for the purpose of review, investigation, or evaluation of a claim or health care operation, to supply each other with information about the health status of, and health care services provided to, me and my applicable Dependent(s). I understand that information disclosed by MedBen to any individuals listed in the preceding paragraph pursuant to this authorization may be subject to redisclosure by such individuals and will no longer be protected by this authorization. This authorization is effective on the date signed and shall remain in effect during while covered and/or insured. (You or any individual authorized by law to act on your behalf have a right to receive a copy of this authorization.) A photographic copy of this authorization shall be as valid as the original. I understand that if I fail to provide this authorization, MedBen will be unable to process my application for coverage, process my claim, or answer my inquiries. I further understand that I have the right to revoke this authorization by submitting such revocation to MedBen’s Chief Privacy Officer at the address listed on this application. Such revocation will not be effective to the extent that action has been taken in reliance upon this authorization prior to MedBen’s receipt of my revocation or to the extent that coverage and/or a claim may be contested under applicable law. I hereby certify that I have personally answered all the questions on this form and that my answers are true and complete to the best of my knowledge and belief. I have legal proof which I can furnish upon request of my relationship to any person listed as a Dependent. I understand that any misstatement(s) or failure(s) to report (including the misstatement of, or failure to report, any medical information) may be used as a basis for rescission or cancellation of insurance for me and my Dependent(s), if any.
No part, portion, or data found on the MedBen Platforms may be reproduced or transmitted in any form or by any means, electronic or mechanical, which includes but is not limited to facsimile transmission, photocopying, recording, re-keying or using any information storage or retrieval system, without express written permission of MedBen.
Your use of the MedBen Platforms constitutes a license, not a transfer in title. You expressly acknowledge and agree that no right is granted to you to copy (other than by printing, downloading, or transmitting portions for your personal use only, but not for other individuals’ use or for commercial exploitation), modify, alter, revise, paraphrase, omit, change, display, perform, store, timeshare, rent lease, sublicense, publish, distribute, transmit, transfer, assign, sell, create derivative works from or commercially exploit in any manner whatsoever any of the content. You may not post, upload, display or otherwise transmit any materials to another website, including but not limited to, any personal or commercial website or home pages.
